Order For Service Form Requirement
(Effective April 3, 2003)

The Order For Service is the formal document stating what services the customer expects us to peform and also outlines our liabilities as an international freight forwarder.

All UniGroup Worldwide UTS Move Managers have been instructed to strictly adhere to policy concerning the issuance of the Order for Service Form. That policy states that no shipment shall be picked up from an origin agent's warehouse unless the St. Louis World Headquarters has received a signed Order for Service.
